Acute - this appointment is for patients that have problems that are in need of care in the next 24 hours, such as fever, abdominal pain, injuries, etc.
Routine - this appointment is for new problems or concerns that can wait for one or two weeks to be evaluated.
Established - this appointment is for follow-up of previously diagnosed or evaluated problems as directed by your provider.
Wellness – this appointment is for annual exams, school or sports physicals and well-baby or child examinations.
Our goal in the clinic is to maintain all your appointments with your assigned provider or Primary Care Manager (PCM). Due to the unpredictability of illnesses, and the rotations of the residents throughout their training, you may be scheduled for appointments with other providers assigned to the same team as your PCM. All routines, wellness and follow-ups will be with your own primary care provider whenever possible. This helps to maintain continuity of care for you and your family.
With provider order - blood pressure checks, periodic medication injections, lab draws, walk-in appointments
Messages for Primary Care Provider/ Medication Refills:
If you need to leave a message for your primary care provider, you can call the clinic during business hours and have a telephone consult sent to your team nurse. Your provider or nurse will respond to this phone call within two duty days. If you feel that it is an emergency, the nurse that answers the call can help you to best manage your situation until the primary care provider can be reached.
Your provider usually gives you enough medication and refills to make it to your next appointment. If you are out of refills, it probably means you are due for a follow-up appointment. If you do need your prescription renewed before your next follow-up appointment, you can call the clinic for a renewal. We ask that you call at least one week before running out of medication. Your provider will enter your script within two duty days and it will be ready to pick up at the main pharmacy after that. FMR
