SHARING Notes

NEW:Payment tracking. Please give the secretaries and the pharmacy the payment log sheets which are located in the SHARING binder. You will enter the data in the statistics at the end of the night.

2. PLEASE emphasize to students that we have many allied health volunteers who are eager to get referrals. If any allied health students are available please pair them with a student provider. DIETETICS: 1st Tuesday of every month. PHYSICAL THERAPY: Second Tuesday of every month. MEDICAL FAMILY THERAPY: no longer come to SHARING. You can refer patients to the day clinic. They also come to every GOODLIFE clinic. OPHTHALMOLOGY: 1st Tuesday of every even-numbered month.

3. Every Woman Matters details: (Mammograms for Female>40 can be referred. Packets can be found to left of secretary desk in a filing cabinet labeled "every woman matters". Yellow form must be filled out by student and patient. Leave on Eileen's desk.

4. Tell students that we are now able to refer patients over the age of 50 to the Nebraska Colon Cancer Screening Program for a colonoscopy. Just indicate this referral on the follow-up and referral form and Eileen will call the patient the next day.

5. Follow-up appointments: Before 7pm, students should bring the patient to the front desk. After 7pm, students just need to fill out "follow-up form" for "X" number of weeks. Eileen (from the day clinic) will call the patient the next day to confirm their next appointment.

6. We are responsible for Stephen's Center patients, if they show up.

7. Smoking Cessation Program Handout: in the patient education cart under "Smoking."

8. If interpretors do not show up by 5:15pm, call Derrick Antoniak at 402-319-9952. If a student provider does not show up, call Kyle Smith at 402-750-1233. If a faculty provider does not show up, call Matt Albin at 402-430-4269.

9. At the end of the night: Charts go on the table in the "chart room," follow-up forms and lab forms go on Eileen's desk (office on the south side), billing sheets and PQRL forms go on the front desk. When entering clinic data at the end of the night make sure you use Internet Explorer.

10. If a student is particularly interested in the care of a certain patient, please have them contact Chris Danford at cdanford@unmc.edu regarding our continuity of care program.

11. When in doubt, fill out BOTH forms for charity care (the colorful brochure, and the long form). The brochure is used for consults while the longer white form is used for consults with a procedure. The patient should fill out the forms and bring them back to the clinic.

12. Please tell students that surgery consults are available. Notify Brandi (bareeve@unmc.edu) when a consult is needed.

13. Please strongly encourage the use of Rx Writer for those who have Carecast training. If students have forgotten their ID or password, they can call the Help Desk at 559-7700.

14. ATTENTION: Strongly remind students that SOAP Notes MUST HAVE proper documentation! This is imperative. The first line must read: "SHARING CLINIC PROGRESS NOTE." The lines must indicate the names of the student and faculty providers.

15. PLEASE Remind students to go over the Medication List with the patient and make any necessary changes in prescription writer. The student also needs to print off a new medication list for the patient.
