  • Target Audience

    Cancer survivors, nurses, caregivers, and others interested in cancer survivorship

    Learning Outcomes
    • Describe cancer survivorship and the importance of each team member's roles during this transition process to improve outcomes.
    • Identify potential long-term effects of cancer treatment and what resources or management strategies are available to develop healthier lifestyles and/or improve quality of life.
    • Articulate information amongst the team including patients, caregivers, nurses, and other health care professionals to ensure a common understanding about post cancer treatment plans and care decisions as derived from the perspectives of the patient and provider.
