MMI Stand-out Nomination Form

MMI Stand-Outs (aka. Employee of the Month Award)
“Why Fit in When You Were Born to Stand Out?” - Dr. Seuss 

WHAT? This award celebrates MMI employees who model ITEACH values (Innovation, Teamwork, Excellence, Accountability, Courage, and/or Healing) while at work or in the community as an MMI representative.

WHO? Anyone (Yes, anyone!) may be nominated or nominate, regardless of title or position. Anyone who works or volunteers at MMI is eligible. Up to five individuals will be selected monthly for this honor.

WHEN? Nominations are due by 8:00 a.m. on the 15th of each month for the next cycle of Stand-Outs. These nominations will be submitted online to Sandy Willett and Janice Flegle for consideration.

WHERE? To nominate, submit this form.

WHY? Because “if you never did, you should. These things are fun, and fun is good.” - Dr. Seuss

Awardees will be notified the first Monday of the month and will be invited to have lunch with Dr. Mirnics. MMI Stand-Outs will receive a “Stand-Out” mug for their morning coffee (or tea) or a water bottle, will have their pictures displayed on the second floor, and will be featured in the MMI Monthly. Congrats already!

Let’s celebrate the extraordinary PEOPLE whose everyday actions make MMI a unique care destination.

In one paragraph (three to five sentences), please explain what this person does to stand-out or why you feel they should be recognized. Please provide specific examples of the person’s actions like: “Barb demonstrated innovation in construction parking management by chasing down the Police Officers who were writing tickets!”; rather than vague compliments like: “Barb is a great innovator!” Your examples may be a single incident or a list of events!
